Gurardian, PPO Dental Plan (Preventative - 100%) ( Basic Restorative - 80%) ( Major Restorative - 50%) Annual Maximum per year - $1000.00 Deductible - $50.00 Noted teeth have silver amalgam fillings that are approx 18-20 yrs old. These teeth each have a crack and have become sensitive to hot and cold foods/liquids. Want to save these teeth rather than extract.
